Insurance
You will pay your specified co-pay amount for each office visit or covered procedure. Our insurance department will file your insurance claim with your health insurance carrier. Please bring your insurance card with you for your first visit. We will make a copy of your card for your file. We may also ask you additional questions about your insurance if we need more information. Please bring your card with you whenever you visit PCA. Coverage may vary from one insurance carrier to another. In most cases, laboratory services, bone densitometry testing, and physical therapy rehabilitation are covered when ordered by your physician. If you have questions about how your insurance is filed, please call our insurance department, 903-408-7750, during business hours. PCA also welcomes self-pay patients. Please call our billing office to arrange payment options.
If you/your child are covered under private insurance (Blue Cross/Blue Shield, Aetna, etc.) there may be additional charges if issues requiring treatment are discovered during a well exam. These issues may be addressed during the well visit or during a follow-up visit. If addressed during the current well exam, an additional E&M code (evaluation and management) will be added to the visit. Your insurance company may require a second co-pay and/or a deductible may apply for the additional service or treatment. You will be charged the co-pay at the time of service and we will bill your insurance company, as usual. If your EOB (explanation of benefits) shows that the charges for additional service or treatment go toward your deductible and are not paid by the insurer, we will bill you for these charges.
